Our telehealth and home visits are direct self-pay, allowing you to receive personalized care without the hassel from insurances. However, all treatments such as medications, and referrals, are billable through your insurance provider when applicable.

New Patient Telehealth appointment - Spine & Weight loss
$75 for a 45-minute session, which includes consultation, comprehensive health assessment, personalized treatment plan, patient portal access, IOS and Android Elation app access.
Follow-Up Telehealth Appointment
$35 for a 30-minute follow-up consultation to assess progress and adjust treatment plans as needed.
New Patient In-Home – Spine Care Visit
$225 for a 60-minute visit, which includes a comprehensive health assessment, physical exam, personalized treatment plan, patient portal access, IOS and Android Elation app access.
Follow-Up In-Home - Physical Exam, Treatment & Referrals
$110 for a 30 minute follow up – post initial telehealth consultation to complete physical examination and personalized treatment plan.
